Enrollment

Thank you for your interest in the First Baptist Head Start Program!

To obtain an application, you can come to our Administration Office at 3890 Railroad Avenue, Pittsburg, CA 94565, or you can contact us at (925) 473-2000.

Once you obtained an application and have filled out the forms, please drop it off at the Administration Office with the following documents in order to start processing:

· Child’s Birth Verification

· Child’s Immunization Record

· Health and Dental Insurance Card

· Family’s Income

· Name of Doctor and Dentist, address and phone #’s

A Family Service representative will review your application and contact you to schedule an appointment for registration if an opening becomes available.  Once you have an appointment scheduled, plan to be here approximately 45 minutes to an hour the day of your appointment, to complete the registration packet.  The Program will also require for your child to have a Physical and Dental examinations, we will give you the forms here at the time of your appointment.
