Thank you for your interest in the First Baptist Head Start Program!
To obtain an application you can come to our Administration Office at 3890 Railroad Avenue, Pittsburg, CA 94565 or you can contact us at (925) 473-2000 and we can fax you an application.
Once you obtain an application and have filled out the forms please drop it off at the Administration Office with the following documents in order to start processing:
· Parents Income
· Child’s Birth Verification
· Child’s Immunization Card
· Health and Dental Insurance Name (if any)
· Health and Dental Insurance Number (if any)
· Name of Doctor and Dentist, address and phone #’s
A Family Service representative will review your application and contact you; if an opening is available, to schedule an appointment for registration. Once you have an appointment scheduled, plan to be here approximately 30-40 minutes the day of your appointment to finish the registration packet. The Program will also require for your child to have a Physical and Dental examinations, we will give you the forms here at the time of your appointment.
